He will be out for five to seven weeks, which means his spell at Bloomfield Road has been officially terminated.
However, Pool boss Simon Grayson does not rule out re-signing the 22-year-old Middlesbrough full-back when the transfer window opens in January.
Grayson said: "It all depends on how many injuries we have and whether Tony is still available.
"He's a good player, who proved in the one full match that he played (the 1-0 victory over Scunthorpe) that he is very capable of doing well in this division.
"It is disappointing for Tony but injuries like this tend to occur with players who have been out a long time, especially after a broken leg like Tony had.
"The good news for us is that we've now got players back who weren't available when we first signed Tony."
