The investigation was triggered by Preston coroner Dr James Adeley, who branded the team ‘unfit for service’ after failing dying Cleveleys woman Dorothy Imisson, 82, in 2014.

He demanded answers after district nurses failed to tell care home staff how to prevent Mrs Imisson, a dementia patient, from developing bed sores that contributed to her death.

The investigation will be finished by Thursday, June 9, hospital documents have now revealed.

The trust, which said it was taking the matter seriously, said it will share its findings with Mrs Imisson’s family.
