Last year’s event, which saw health workers from the NHS and private sector honoured in a glittering ceremony at Ribby Hall Village in Wrea Green, was such a success, this year’s awards promises to be even bigger and better.
The awards have been opened up to cover more areas of health, and the presentation ceremony for 2012 will be even more prestigious.
We are asking you, our readers, to nominate the person from the health sector you feel has made an outstanding contribution, has gone the extra mile, or done something above and beyond the call of duty.
We are looking for those who are the best in their field, whether they be well-known and high-profile workers, or the unsung heroes who work tirelessly behind the scenes.
Perhaps you have a family member who devotes their time to caring for a loved one, or you know someone in your community who looks after the health and welfare of others.
The awards aim to recognise excellence and innovation in health professionals, but also to honour those who care in the voluntary sector.

Gazette deputy editor Alison Bott said: “We are looking for those who genuinely represent the best in health – whether from the voluntary or paid-for sector.
“These awards will serve to act as recognition and as a reward, to those who go the extra mile to look after the health of others.
“Last year’s awards were such a great success and we heard some truly inspiring and heart-warming stories of those who really are health heroes.”

The judging panel will shortlist nominees in each category, from which it will choose one winner. You can nominate by emailing suzanne.steedman@blackpoolgazette.co.uk, or in writing to Suzanne Steedman at The Gazette, Avroe House, Avroe Crescent, Blackpool Business Park, Blackpool FY4 2DP.
You will need to provide the name and contact details of the person you are nominating, along with your own name and contact details and your reasons for nominating that person in no more than 300 words. Closing date for entries is March 28.
Carer of the Year

For anyone who cares and gives their time and their all – either voluntary or in the paid-sector – for the benefit of others. It could be someone who works in a care home, a social worker or someone who works for social care services, or could be a local resident who looks after an ill, disabled or infirm relative, friend or neighbour.
Dentist of the Year
For the dentist, either from the NHS or from the private sector, who goes the extra mile.
Doctor of the Year

For the GP, family doctor, hospital doctor, registrar or consultant, from the NHS or private sector – who goes above and beyond the call of duty to provide the best service possible for patients.
Unsung hero Award
As sponsored by Barchester - Care Homes, Residential & Day-care nursing Homes for the Elderly:
For the person who has consistently gone that extra mile to provide the best service possible for patients. For example, you might want to nominate the hospital porter who always greets patients with a smile, the care home cleaner who makes sure everything is spick and span, the pharmacist who offers advice with minor condition and medication, or the doctors’ receptionist, who is always friendly and approachable, or simply someone in the community who looks after the health and welfare of others.
Young Unsung hero Award

For the person under the age of 21, who consistently goes the extra mile to help care for the health of others.
Nurse of the Year
As sponsored by The Priory Hospital Quayside House– specialist in mental health issues:
For the nurse, who works for a hospital, or GP surgery, in the NHS, in the private sector or in the community, who rises above expectations in their care for patients.
Midwife of the Year
For the midwife who works for the hospital trust, in the community or in the private sector, who goes above and beyond the call of duty for mums-to-be and their babies
Optician of the Year

For the optician, optometrist or ophthalmologist, either working in the NHS or private sector, who goes the extra mile for patients.
Elderly Welfare Award

For the person – whether they be an unpaid volunteer or a worker, whether they work in a nursing home, in social services – who devotes their time and energy to the welfare of the elderly.
Child Care Award
